In exchange of participation in classroom activities provided by Art of Inquiry, and use of the property, facilities and services of Art of Inquiry, I agree for myself and ( if applicable), for the members of my family, to the following:
1. I agree to observe and obey all rules and warnings, and follow any oral instructions or directions given by representatives of Art of Inquiry.
2. I assume full responsibility for personal injury to myself and if applicable, to the members of my family, and further release and discharge Art of Inquiry for any injury, loss or damage arising out of my (or my family’s) use of or presence upon the facilities (or rented facilities) of Art of Inquiry, whether caused by the fault of myself, my family, Art of Inquiry or its representatives, or other third parties.
3. I agree to indemnify and defend Art of Inquiry against all claims, causes of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my (or my family’s) use of or presence upon the facilities ( or rented facilities) of Art of Inquiry.
4. I agree to pay for all damages to the facilities ( or rented facilities) of Art of Inquiry caused by my (or my family’s) negligent, reckless or willful actions.
5. I consent to the participation of my child (ren) in Art of Inquiry classroom activities, and agree on behalf of the the mentioned child(ren) to all of the terms and conditions of this Agreement. By signing this Release of Liability, I represent that I have legal authority over and custody of my child (ren) named below.
6. In the event of an injury to the me or my family members, I give my permission to Art of Inquiry or to its representatives to arrange for all necessary medical treatment for which I will be financially responsible. |
